2FA Duo Token Request

Please review the Libraries’ Guidelines for Requesting Tokens for Two Factor Authentication (2FA):

Tokens will only be purchased if access to WebAccess is required in the course of doing university business.
Tokens will be available on request only.
Requests should be made by individuals via this form.
Tokens must be approved by the Senior Director of Administrative and Financial Services or designee.
Tokens are managed by the Libraries Business Office.
Tokens are assigned to individuals and must not be shared or transferred.
In the event an employee leaves employment of the Penn State Libraries, the token is returned to Libraries Business Office.
Lost tokens are the responsibility of the employee, and the employee is responsible for replacement costs.
